However, the facts note caution for people who are very ill or immunocompromised — there have been rare but serious cases of bacteremia (bacteria in the bloodstream) and sepsis linked to these organisms in critically ill infants, preterm infants, and adults with weakened immunity.
Common side effects are uncommon but can include bloating, flatulence, abdominal discomfort, and rarely diarrhea or constipation. One case of headache was reported in a trial.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
