Why this applies to Ertapenem
Ertapenem falls under ANTIBIOTIC DRUGS — the group Bacillus Subtilis interacts with.
Since B. subtilis preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. subtilis preparations by at least 2 hours.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to keep in mind: this interaction works against the probiotic, not against your antibiotic. Ertapenem (Invanz) is a powerful carbapenem antibiotic that kills bacteria, and Bacillus subtilis is itself a live bacterium. So if the two cross paths at the same time, the ertapenem can simply wipe out a good chunk of those probiotic organisms, leaving you with little benefit from the supplement.
The good news is your ertapenem keeps working normally, so you don't lose any infection-fighting power. This concern is mostly theoretical, based on how the two behave. If you want to take both, a simple fix is spacing them apart. Since ertapenem is given as an injection, ask your pharmacist or doctor how best to time your Bacillus subtilis dose around it.

Ertapenem treats moderate to severe infections caused by susceptible bacteria in adults and children 3 months and older. These include complicated infections in the abdomen, skin and skin structures (including diabetic foot infections without osteomyelitis, a bone infection), lungs (community-acquired pneumonia), urinary tract (including pyelonephritis, a kidney infection) and pelvis.
In adults only, it is also used to help prevent surgical site infection after elective colorectal surgery. It should be used only for infections that are proven or strongly suspected to be bacterial.
