Exert caution when fluconazole is coadministered with lovastatin (CYP3A4 substrate and HMG-CoA reductase inhibitor) as it may increase lovastatin exposure and may also increase the risk of myopathy and rhabdomyolysis. Hence, if concomitant therapy is necessary, monitor patients carefully and observe for symptoms of myopathy and rhabdomyolysis and also monitor creatinine kinase. Discontinue lovastatin if a marked increase in creatinine kinase is observed or myopathy/rhabdomyolysis is diagnosed or suspected. Dose reduction of statins may also be needed12.
