Here's the simple version: both Sweet Annie and lovastatin can occasionally be tough on your liver. Lovastatin already carries a small risk of liver irritation, which is why your doctor sometimes checks liver enzymes with bloodwork. Sweet Annie (the herb itself) has also been linked to possible liver stress in a few reports. Put them together and the worry is that those effects could add up.

I want to be honest about the evidence here, though. This is mostly theoretical and based on scattered, anecdotal reports, not strong studies. So it isn't a reason to panic. Just watch for signs like unusual tiredness, yellowing of your skin or eyes, dark urine, or pain in the upper right belly, and check with your pharmacist or doctor before combining them.

Lovastatin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the situation with lovastatin and Sweet Annie. Your body relies on an enzyme called CYP3A4 to clear lovastatin, and Sweet Annie may tug on that enzyme. The tricky part is the direction isn't settled. In lab dishes, the artemisinin in Sweet Annie sped this enzyme up, while the whole-plant extract actually slowed it down. So the real effect in a person is genuinely unclear.

If the enzyme speeds up, lovastatin could clear faster and do less for your cholesterol. If it slows down, lovastatin can build up, raising the odds of muscle aches, weakness, or rare but serious muscle breakdown. This is only test-tube evidence, so don't panic. Just loop in your pharmacist or doctor before pairing them.

While the purified drug is well studied for malaria, the herb itself as a supplement is not proven for the wide range of conditions people use it for, and self-treating malaria with the raw herb is dangerous. About Lovastatin

What Is Lovastatin Used For?

Brand names: Mevacor, Altocor

Lovastatin is used to lower high LDL ("bad") cholesterol and total cholesterol in people whose levels have not improved enough with diet alone. It is also used to slow the buildup of plaque in the arteries (coronary atherosclerosis) in people who already have coronary heart disease.

In adults without existing heart disease but with high cholesterol and low HDL ("good") cholesterol, lovastatin tablets are approved to reduce the risk of heart attack, unstable angina, and the need for procedures to open blocked arteries. Lovastatin tablets are also approved for adolescents aged 10–17 with a specific inherited condition called heterozygous familial hypercholesterolemia (heFH), when diet changes alone haven't been enough.
