Why this applies to Fluconazole

Fluconazole falls under HEPATOTOXIC DRUGS — the group Ostarine interacts with.

Theoretically, concomitant use with hepatotoxic drugs might increase the risk of adverse hepatotoxic effects.
Some clinical research shows that ostarine can increase alanine aminotransferase, a marker of liver damage, in some patients. Additionally, the U.S. Food and Drug Administration warns that supplements containing SARMs, such as ostarine, have been associated with reports of liver toxicity and there are at least two reports of drug-induced liver injury attributed to the use of ostarine.

Here's the thing to keep in mind with this pair: it isn't about one changing the levels of the other. Instead, both Ostarine and Fluconazole can be hard on the liver, so using them together may stack that risk. Fluconazole on its own can sometimes raise liver enzymes, and Ostarine has been tied to reports of liver injury and rising ALT (a marker of liver stress).

I want to be honest about the evidence here, it's mostly theoretical and based on how each behaves rather than studies of the two together. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or feeling unusually tired. Since Ostarine is an unapproved SARM with real safety concerns, please loop in your doctor or pharmacist before combining them.

Fluconazole also falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS.

Theoretically, concomitant use of ostarine with CYP3A4 inhibitors could increase the effects and adverse effects of ostarine, although this is unlikely to be clinically significant.
Ostarine is partially metabolized by CYP3A4. However, clinical research shows that taking itraconazole, a potent inhibitor of CYP3A4, had minimal effects on the levels of ostarine in the body.

Here's the gist: Fluconazole slows down a liver enzyme called CYP3A4, and Ostarine is partly broken down by that same enzyme. In theory, blocking it could let Ostarine build up a bit, which might mean stronger effects and a higher chance of its known downsides, like liver strain. So the concern points toward more Ostarine in your system, not less.

In real life, though, this looks weak. When researchers paired Ostarine with itraconazole (an even stronger CYP3A4 blocker than fluconazole), the Ostarine levels barely budged. That tells us the interaction is mostly theoretical. The bigger worry is Ostarine itself, since it's an unapproved SARM tied to liver injury. Please loop in your pharmacist or doctor before mixing the two.

It is not approved by the FDA, is banned in sports, and has been linked to liver injury and other serious harms, so it is not recommended for general use.

About Fluconazole

What Is Fluconazole Used For?

Brand names: Diflucan, Diflucan Injection

Fluconazole treats infections caused by Candida and Cryptococcus fungi. Diflucan tablets and oral suspension, fluconazole tablets, and fluconazole oral suspension and tablets are approved for vaginal yeast infections (vaginal candidiasis), mouth and throat yeast infections (oropharyngeal candidiasis), and esophageal candidiasis. All formulations, including the intravenous injection, are used for cryptococcal meningitis (a fungal infection of the brain lining). In smaller studies, fluconazole was also effective for Candida urinary tract infections, peritonitis (abdominal lining infection), and serious systemic infections such as candidemia (Candida in the bloodstream) and disseminated candidiasis.

Fluconazole is also used as preventive therapy: Diflucan, fluconazole tablets, fluconazole oral suspension, and fluconazole injection are all indicated to reduce the risk of candidiasis in patients undergoing bone marrow transplantation who receive chemotherapy and/or radiation therapy. Note that the oral suspension formulations are not approved for vaginal candidiasis — that indication applies only to Diflucan tablets/suspension and fluconazole tablets/tablet-suspension products.
