Rifampin falls under HEPATOTOXIC DRUGS — the group Ostarine interacts with.

Theoretically, concomitant use with hepatotoxic drugs might increase the risk of adverse hepatotoxic effects.
Some clinical research shows that ostarine can increase alanine aminotransferase, a marker of liver damage, in some patients. Additionally, the U.S. Food and Drug Administration warns that supplements containing SARMs, such as ostarine, have been associated with reports of liver toxicity and there are at least two reports of drug-induced liver injury attributed to the use of ostarine.

Here's the concern with pairing these two: rifampin is one of those medicines that can already be tough on your liver, and it's standard to keep an eye on liver tests while you're on it for TB. Ostarine has its own track record of liver trouble, including real-world reports of liver injury and rising ALT levels (a blood marker that signals liver stress). Stacking two things that each stress the liver may add up.

This is a theoretical concern based on how both act, not proof that the combo harms everyone. Still, watch for warning signs like yellowing of the skin or eyes, dark urine, belly pain, nausea, or unusual tiredness. Rifampin already turns urine reddish-orange normally, so don't confuse that with a problem. Please talk with your doctor or pharmacist before combining these.

Ostarine is partially metabolized by CYP3A4. Clinical research shows that taking rifampin, a potent inducer of CYP3A4, reduces the maximum plasma concentration of ostarine by 23% and the area under the curve by 43%.

Here's the simple version: Rifampin is one of the strongest "speed-up" drugs we know of. It revs up a liver enzyme called CYP3A4, which is one of the pathways that breaks down Ostarine. In real research, taking rifampin dropped Ostarine's blood levels noticeably (peak levels down about 23%, total exposure down about 43%). So the concern here is that rifampin would clear Ostarine out faster, leaving less of it in your system and weaker effects, not more side effects.

Honestly though, my bigger worry is Ostarine itself. It's an unapproved, banned SARM that has been tied to liver injury, and rifampin can also stress the liver. I'd strongly encourage you to talk with your doctor or pharmacist before combining them, and ideally reconsider the Ostarine altogether.

Here's the thing to understand: in this pairing, Rifampin is the medicine that does the affecting, not the one that gets affected. Rifampin is a strong "inducer," meaning it revs up a liver enzyme called CYP3A4. Ostarine is partly broken down by that same enzyme, so when the two are taken together, your body clears the Ostarine faster. Studies show the blood levels of Ostarine dropped by roughly a quarter at peak and nearly half overall, so the SARM would likely work weaker.

Rifampin itself isn't made stronger or weaker here, so your TB or meningitis treatment shouldn't change. That said, I'd gently remind you that Ostarine is an unapproved, banned substance linked to liver injury, and Rifampin can stress the liver too. Please loop in your doctor or pharmacist before mixing anything with your Rifampin.

About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
