Why this applies to Cyclosporine

Cyclosporine falls under HEPATOTOXIC DRUGS — the group Ostarine interacts with.

Theoretically, concomitant use with hepatotoxic drugs might increase the risk of adverse hepatotoxic effects.
Some clinical research shows that ostarine can increase alanine aminotransferase, a marker of liver damage, in some patients. Additionally, the U.S. Food and Drug Administration warns that supplements containing SARMs, such as ostarine, have been associated with reports of liver toxicity and there are at least two reports of drug-induced liver injury attributed to the use of ostarine.

Here's the concern with pairing these two. Cyclosporine is an immune-suppressing medicine that, on its own, can be tough on the liver. Ostarine (a SARM that isn't an approved supplement) has also been tied to liver injury, and some research shows it can bump up ALT, a blood marker that signals liver stress. Stack two liver-stressing agents together and you may raise the odds of trouble like fatigue, nausea, belly pain on the right side, dark urine, or yellowing of the skin or eyes.

To be fair, this is a theoretical caution based on how each one behaves, not proven harm from combining them. Still, with cyclosporine your doctor already watches liver and kidney labs closely, so please loop in your pharmacist or transplant team before using Ostarine.

Cyclosporine also falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS.

Theoretically, concomitant use of ostarine with CYP3A4 inhibitors could increase the effects and adverse effects of ostarine, although this is unlikely to be clinically significant.
Ostarine is partially metabolized by CYP3A4. However, clinical research shows that taking itraconazole, a potent inhibitor of CYP3A4, had minimal effects on the levels of ostarine in the body.

Here's how this one works: cyclosporine can act as a mild blocker of an enzyme called CYP3A4, and Ostarine is partly broken down by that same enzyme. In theory, slowing that pathway could let a little more Ostarine build up, which might mean stronger effects and a higher chance of its known harms, like liver strain. The good news is that this interaction is rated minor and unlikely. Real research using a much stronger CYP3A4 blocker (itraconazole) barely moved Ostarine levels at all, so the practical risk looks small.

That said, I want to be honest with you: cyclosporine itself is a serious, narrow-margin transplant and immune medicine, and Ostarine is an unapproved SARM that I can't recommend. Please loop in your doctor or pharmacist before mixing anything with cyclosporine.

Cyclosporine is prescribed to help prevent the body from rejecting a transplanted kidney, liver, or heart. It is often used alongside other immunosuppressive medicines as part of a full transplant care plan.

It is also used for adults with severe, active rheumatoid arthritis that hasn't responded well enough to methotrexate, and for adults with severe plaque psoriasis who haven't had success with other standard treatments.
