Ostarine and Dronabinol

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Why this applies to Dronabinol

Dronabinol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ostarine interacts with.

Theoretically, ostarine might increase levels of drugs metabolized by CYP2C9, although clinical research suggests this is not clinically relevant.
Although in vitro research suggests that ostarine inhibits CYP2C9, more robust clinical research shows that ostarine does not significantly affect CYP2C9.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Dronabinol (Marinol, Syndros) and Ostarine. Your body uses a liver enzyme called CYP2C9 to help clear dronabinol. In a test tube, Ostarine looked like it might slow that enzyme down, which in theory could let dronabinol build up a bit and bring on stronger effects like drowsiness, dizziness, a racing or pounding heart, confusion, or that overly "high" feeling. Dronabinol is the active drug already, not a prodrug, so more of it simply means more effect.

The reassuring part: better human research suggests Ostarine doesn't actually change CYP2C9 enough to matter here, so this is rated minor and unlikely. Still, Ostarine is an unapproved SARM tied to liver harm, so I'd talk it over with your pharmacist or doctor before combining anything.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Dronabinol page

“A glass of grapefruit juice can push dronabinol (Marinol, Syndros), the prescription form of THC used for chemotherapy nausea and appetite loss, to higher blood levels than intended, amplifying dizziness, sedation, and its mind-altering effects. That is one of three major-rated interactions among the 151 supplements and herbs on file,…”

About Ostarine

What is Ostarine?

  • Muscle building
  • Athletic performance
  • Fat loss / body recomposition
  • Bodybuilding cycles

Ostarine (also called MK-2866 or enobosarm) is an experimental, unapproved drug known as a SARM (selective androgen receptor modulator), not a true dietary supplement. It is not approved by the FDA, is banned in sports, and has been linked to liver injury and other serious harms, so it is not recommended for general use.

About Dronabinol

What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
