Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Acacia Rigidula interacts with.

Theoretically, Acacia rigidula products that are adulterated with beta-methylphenylethylamine (BMPEA) might increase the levels of CYP3A4 substrates.
In vitro research shows that BMPEA, an adulterant found in many Acacia rigidula products, inhibits CYP3A4 enzyme by 37%.

Here's the short version on mixing Acacia rigidula with bedaquiline (Sirturo). Your liver uses an enzyme called CYP3A4 to clear bedaquiline out of your body. Some Acacia rigidula products are spiked with a stimulant called BMPEA, and in lab (test-tube) studies BMPEA slowed that enzyme down by about a third. If that happened in a real person, bedaquiline could build up a bit higher than expected.

Why that matters: bedaquiline can affect your heart rhythm (a QT prolongation effect), so higher levels could theoretically raise that risk, along with nausea or liver irritation. Keep in mind this is only theoretical lab evidence, not proven in people. Still, with a drug this important for treating TB, don't add this supplement without first checking with your TB doctor or pharmacist.

About Bedaquiline

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
