Here's the honest picture for valproate (Epilim) and acetyl-L-carnitine. This particular interaction is flagged because some animal studies suggest acetyl-L-carnitine may nudge up serotonin levels in the brain. The concern is that adding it to a drug with serotonin-related activity could, in theory, raise the chance of serotonin syndrome, which can show up as agitation, sweating, fast heartbeat, shivering, or muscle twitching.

I want to be straight with you though: this is a theoretical worry based only on lab and animal data, not real-world human reports, so the actual risk for most people taking these two together is likely low. Valproate isn't a prodrug, and this isn't about how it's broken down. Still, since everyone's situation differs, please run it past your pharmacist or doctor before starting acetyl-L-carnitine.
