Why this applies to Aliskiren

Aliskiren falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group African Wild Potato interacts with.

In vitro evidence shows that African wild potato extract can inhibit cytochrome P450 3A4 (CYP3A4) enzyme activity by up to 86%. Theoretically, African wild potato might increase levels of drugs metabolized by CYP3A4. However, in human research, African wild potato does not seem to affect the pharmacokinetics of Efavirenz, a CYP3A4 substrate. Until more is known, use African wild potato cautiously or avoid in patients taking drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the short version: your blood pressure pill aliskiren (Tekturna) is partly broken down by a liver enzyme called CYP3A4. In a test tube, African wild potato slowed that enzyme down quite a bit. The worry is that if the enzyme works slower, less aliskiren gets cleared and more of it lingers in your body, which could nudge its blood pressure lowering effect up a notch.

If that happened, you might notice dizziness, lightheadedness, or feeling faint, especially when standing up. But I want to be honest with you: this is based on lab data only, not real people, and in one human study this plant didn't change a similar drug. So it's a theoretical concern, not a proven one. Still, run it by your pharmacist or doctor before combining them.

What Is Aliskiren Used For?

Aliskiren tablets — available as the brand Tekturna and as generic aliskiren — are used to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering blood pressure reduces the risk of strokes and heart attacks.

Controlling blood pressure is most effective as part of a broader plan that may include healthy eating, exercise, reducing sodium, and managing other risk factors like cholesterol and blood sugar. Many people need more than one blood pressure medicine to reach their goal.
