Astaxanthin and Tadalafil

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Tadalafil

Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Astaxanthin interacts with.

Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP3A4.
In vitro research shows that astaxanthin induces CYP3A4 enzyme activity in human hepatocytes. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist if you take tadalafil (Cialis) and want to add astaxanthin. Your body clears tadalafil using a liver enzyme called CYP3A4. In lab studies using human liver cells, astaxanthin seemed to speed up that enzyme a bit. If that happened in a real person, tadalafil could be broken down faster and not work as well, so you might notice a weaker response for ED or your BPH symptoms.

The key thing is this is only test-tube data and has never actually been shown in people, so I wouldn't lose sleep over it. Tadalafil isn't a prodrug, so the direction makes sense (faster clearance, less effect). Before starting astaxanthin, just run it by your pharmacist or doctor so they can keep an eye on how well your tadalafil is working.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

These ratings are for information only. Never start, stop, or otherwise change your therapy before speaking with your provider.

These are real questions from readers, answered by the pharmacists at HelloPharmacist — written by a person, not generated from a database. This page covers Astaxanthin only. If you take other supplements — a multivitamin, a sleep aid, something for joints — the Tadalafil page lists 144 supplements it has a documented interaction with, worst first, each one explained.

The most serious there right now: Activated Charcoal (major), American Mistletoe (major), Grapefruit (major), Hawthorn (major) and St. John's Wort (major).

By severity: 5 major · 98 moderate · 41 minor.

From our pharmacist’s overview on the Tadalafil page

“Grapefruit juice is the first thing to know about if you take tadalafil (Cialis, Tadliq). It blocks CYP3A4, the enzyme that clears the drug, so blood levels climb and side effects like a steep drop in blood pressure become more likely.”

About Astaxanthin

What is Astaxanthin?

Astaxanthin
  • Antioxidant support
  • Skin health and aging
  • Eye health
  • Exercise recovery
  • Heart and inflammation support

Astaxanthin is a reddish carotenoid pigment with strong antioxidant activity in the lab, and it is widely promoted for skin, eye, heart, and exercise benefits. Early human studies are promising for a few uses, but the overall evidence is still limited and not strong enough to make firm claims. It appears generally well tolerated at typical supplement doses, but talk with your pharmacist or doctor before starting it.

About Tadalafil

What Is Tadalafil Used For?

Brand names: Cialis, Tadliq

Tadalafil tablets (including Cialis and generic tadalafil) are used to treat erectile dysfunction (ED), the symptoms of benign prostatic hyperplasia (BPH — an enlarged prostate), or both conditions together. When used with the drug finasteride to start BPH treatment, this combination is recommended for up to 26 weeks.

Adcirca, Alyq (tablets), and Tadliq (oral suspension) are indicated specifically for pulmonary arterial hypertension (PAH — high blood pressure in the lungs), to improve the ability to exercise.
