Why this applies to Darunavir, Ritonavir
Darunavir, Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bael interacts with.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use bael cautiously or avoid in patients taking these drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with this one. Both darunavir and ritonavir are handled by a liver enzyme called CYP3A4, and in fact ritonavir is often added on purpose to block that enzyme so darunavir stays in your system longer. In lab dishes, compounds in bael also slowed CYP3A4 down. In theory, that means bael could nudge the levels of these HIV medicines a bit higher, which might raise the chance of side effects like nausea, diarrhea, headache, or changes in your blood sugar and cholesterol.
But keep this in perspective. This was only seen in test-tube studies, never in real people, and it's rated a minor, possible interaction. Eating bael as a food likely won't matter much, though concentrated supplements are less predictable. Since keeping HIV therapy steady is important, just run it past your pharmacist or doctor first.
