Ciprofloxacin, Hydrocortisone falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Banaba interacts with.

Theoretically, concomitant use of banaba with substrates of OATP might reduce the bioavailability of the OATP substrate.
In vitro research shows that banaba inhibits OATP, particularly OATP2B1. OATPs are expressed in the small intestine and liver and are responsible for the absorption of drugs and other compounds.

Here's some reassuring context first: the product you're asking about, Cipro HC Otic, is an ear drop. The ciprofloxacin and hydrocortisone work right where you place them, in the ear canal, and very little gets into your bloodstream. So even though banaba may block a gut and liver transporter called OATP2B1 in lab studies, that effect is about how much of an oral drug your body absorbs. With drops in your ear, absorption through the gut simply isn't part of the picture.

On top of that, this interaction is only theoretical and based on test-tube research, not real people. For your ear drops, I wouldn't expect any meaningful problem. If you ever take oral ciprofloxacin instead, that's worth running by your pharmacist.
