Why this applies to Fluvastatin

Fluvastatin falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Banaba interacts with.

Theoretically, concomitant use of banaba with substrates of OATP might reduce the bioavailability of the OATP substrate.
In vitro research shows that banaba inhibits OATP, particularly OATP2B1. OATPs are expressed in the small intestine and liver and are responsible for the absorption of drugs and other compounds.

Here's the basic idea with these two together. Fluvastatin (Lescol) relies on transporters called OATPs to get absorbed from your gut and taken up into your liver, which is exactly where it does its cholesterol-lowering work. Lab research suggests banaba may block these transporters, especially one called OATP2B1. If that happened in real life, less fluvastatin could reach the right places, which theoretically means a weaker effect on your LDL (bad cholesterol).

I want to be honest about the strength of this: it comes only from test-tube studies, not from people, so we really don't know if it matters in practice. Fluvastatin isn't a prodrug, so the concern is reduced benefit rather than extra side effects. Don't stop either one on your own. Just mention the banaba to your pharmacist or doctor so they can keep an eye on your cholesterol numbers.

About Fluvastatin

What Is Fluvastatin Used For?

Brand names: Lescol, Lescol XL

Fluvastatin capsules are used alongside diet to lower total cholesterol, LDL cholesterol, triglycerides, and apolipoprotein B, and to raise HDL cholesterol in adults with primary hypercholesterolemia (high cholesterol) or mixed dyslipidemia (abnormal cholesterol and triglyceride levels). Fluvastatin capsules are also used in boys and girls aged 10–16 who have heterozygous familial hypercholesterolemia (an inherited form of high cholesterol) when diet alone has not worked. In adults with clinically evident coronary heart disease (CHD), fluvastatin capsules may reduce the risk of needing a coronary revascularization procedure (a procedure to restore blood flow, such as angioplasty) and can slow the progression of coronary atherosclerosis (plaque buildup in heart arteries).

Fluvastatin sodium extended-release tablets and Lescol XL (extended-release tablet) share similar uses: reducing LDL cholesterol in adults with primary hyperlipidemia, reducing LDL in adults and children aged 10 and older with heterozygous familial hypercholesterolemia who require the highest available dose, and reducing cardiovascular risk in adults with established coronary heart disease. Fluvastatin is always used together with a cholesterol-lowering diet, not as a replacement for one.
