Grepafloxacin falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Banaba interacts with.

Theoretically, concomitant use of banaba with substrates of OATP might reduce the bioavailability of the OATP substrate.
In vitro research shows that banaba inhibits OATP, particularly OATP2B1. OATPs are expressed in the small intestine and liver and are responsible for the absorption of drugs and other compounds.

Here's the gist: grepafloxacin is an antibiotic, and your body relies partly on transporters called OATPs to absorb it from the gut. Lab studies suggest banaba may block one of these transporters (OATP2B1). If that happens in real life, less of your antibiotic could get absorbed, meaning lower blood levels and a weaker punch against the infection you're trying to clear.

I want to be honest about how thin this evidence is. It comes only from test-tube research, not from people, so we genuinely don't know if it matters at the doses you'd take. Grepafloxacin itself is no longer widely used, but as a general rule, when you're on any antibiotic, you don't want anything quietly reducing how well it works. Please run this past your pharmacist or doctor before combining them.
