Why this applies to Methadone
Methadone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Beet interacts with.
In vitro research suggests that betanin, the major pigment in beet, competitively inhibits CYP3A4 in a dose-dependent manner similarly to strong CYP3A4 inhibitor ketoconazole.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for methadone: your liver uses an enzyme called CYP3A4 to help break it down. Some lab research suggests betanin, the deep red pigment in beets, can slow that same enzyme. If that happened in your body, methadone could clear out a little more slowly and build up, which means stronger effects like drowsiness, slowed breathing, or constipation. Methadone is a drug where even modest changes deserve respect, so this is worth noting.
That said, this is only a test-tube finding, not something proven in people, and normal amounts of beets in food are unlikely to cause trouble. Concentrated beet supplements are the bigger question mark. Please check with your pharmacist or doctor before adding them, and don't change your methadone on your own.

It is generally safe as a food, but supplement effects are modest and not a substitute for prescribed treatment, so talk with your pharmacist or doctor before relying on it.

What Is Methadone Used For?

Methadone treats two main problems. Some products are used for severe, persistent pain that needs daily, around-the-clock opioid treatment when other options are not adequate. Some products are used for opioid addiction (heroin or other morphine-like drugs), as detoxification or as maintenance treatment along with social and medical services.
Not every product is approved for every use. Methadone tablets, oral solution and oral concentrate (Intensol) list both pain and opioid addiction. Diskets, Methadose Dispersible tablets and the Methadose and Methadone Hydrocloride oral concentrates are labeled only for opioid addiction. Methadone injection is for severe pain, and for short-term treatment of opioid dependence in people who cannot take medicine by mouth, such as hospitalized patients. It is not approved for outpatient opioid dependence treatment. Methadone is not meant to be used as an as-needed pain reliever.
