Aspirin, Caffeine, Orphenadrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

This combo pill pairs aspirin and caffeine with orphenadrine, a muscle relaxant. Orphenadrine is partly broken down by a liver enzyme called CYP3A4. Berberine can mildly slow that same enzyme down. In theory, that means orphenadrine could clear a little more slowly and build up in your body, which may bump up its side effects like drowsiness, dry mouth, blurred vision, dizziness, or a faster heartbeat. Caffeine on its own can also make you feel more jittery.

To be fair, this concern is mostly theoretical and based on lab and early human data, so for many people it stays minor. Still, I'd run it past your pharmacist or doctor before adding berberine, especially if you already feel groggy on this medicine.

Theoretically, berberine might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, berberine might have additive effects when used with anticoagulant and antiplatelet drugs and increase the risk of bleeding.

The piece of this combo pill that matters here is the aspirin. Aspirin already makes your platelets less sticky, which is how it helps prevent clots but also why it can cause bruising or bleeding. Berberine has been shown in lab and animal studies to also slow platelets down a bit. So in theory, taking them together could add up and nudge your bleeding risk a little higher.

I want to be honest though: this is a theoretical concern based on test-tube and animal research, not strong human studies. Still, keep an eye out for things like easy bruising, nosebleeds, bleeding gums, or dark stools. Before adding berberine, check with your pharmacist or doctor so they can weigh it against your situation.
