Disopyramide Phosphate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the gist for Norpace (disopyramide). Your body clears this heart-rhythm drug largely through a liver enzyme called CYP3A4. Berberine appears to slow that enzyme down a bit, so in theory disopyramide could clear more slowly and build up higher than expected in your blood.

That matters because disopyramide is a narrow-margin medicine. Too much can bring on dry mouth, blurred vision, trouble urinating, low blood pressure, or even new rhythm problems like QT prolongation. The evidence here is mostly lab-based and theoretical, not proven in people taking these two together, so I don't want to alarm you. But because this is a heart drug, please don't add berberine without first checking with your pharmacist or cardiologist.

What Is Disopyramide Used For?

Brand names: Norpace CR, Norpace

Disopyramide — available as Disopyramide Phosphate, Norpace, and Norpace CR — is indicated for the treatment of documented ventricular arrhythmias, such as sustained ventricular tachycardia (a fast, dangerous heart rhythm originating in the lower chambers), that a physician judges to be life-threatening.

Because of its potential to worsen heart rhythm problems, it is generally not recommended for lesser arrhythmias, and it should not be used to treat ventricular premature contractions (extra heartbeats) that cause no symptoms. Treatment is typically started in a hospital setting.
