Itraconazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the gist with itraconazole and berberine. Your body clears itraconazole using a liver enzyme called CYP3A4. Berberine appears to slow that same enzyme down a bit, so the worry is that itraconazole could build up to higher levels than usual. That can mean a greater chance of side effects like nausea, stomach upset, headache, or, less commonly, swelling in the legs or changes in liver function.

To be fair, this is mostly a theoretical concern based on lab work and early research, so it may not cause noticeable trouble for everyone. Still, itraconazole already interacts with a lot of things, so I'd run the combination past your pharmacist or doctor before starting berberine rather than guessing.

About Itraconazole

What Is Itraconazole Used For?

Brand names: Sporanox, Sporanox Injection, Tolsura

Itraconazole capsules treat several serious fungal infections: blastomycosis, histoplasmosis, and aspergillosis (in people who cannot tolerate or have not responded to amphotericin B). Sporanox and generic itraconazole capsules are also used for toenail and fingernail fungus (onychomycosis) in people with normal immune systems.

Itraconazole oral solution has a different use: it treats oropharyngeal and esophageal candidiasis (yeast infection, or thrush, of the mouth, throat and esophagus). Tolsura capsules are approved only for the three serious infections in adults, not for nail fungus.
