Ivosidenib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the gist with Tibsovo (ivosidenib) and berberine. Your liver relies on an enzyme called CYP3A4 to clear ivosidenib out of your body. Berberine appears to slow that same enzyme down, so the worry is that ivosidenib could build up to higher levels than intended. With this cancer medicine, more in your bloodstream can mean a greater chance of side effects.

One thing I'd especially flag: ivosidenib can affect your heart's rhythm (a QT prolongation effect on the EKG), and higher drug levels could make that more likely. You might also see more nausea, fatigue, or joint pain. This concern is mostly theoretical for now, but ivosidenib is a serious drug, so please don't add berberine without first clearing it with your oncologist or pharmacist.

About Ivosidenib

What Is Ivosidenib Used For?

Brand name: Tibsovo

Tibsovo (ivosidenib) is used to treat adults who have a susceptible IDH1 mutation, confirmed by an FDA-approved test. It treats newly diagnosed acute myeloid leukemia (AML) in adults 75 or older, or those who cannot tolerate intensive chemotherapy — either alone or combined with a medicine called azacitidine. It also treats relapsed or refractory AML (AML that has come back or stopped responding to treatment) and relapsed or refractory myelodysplastic syndromes (MDS).

Tibsovo is also approved for adults with previously treated, locally advanced or metastatic cholangiocarcinoma (bile duct cancer that has spread) who carry an IDH1 mutation.
