Olanzapine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP2D6.
In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, use of berberine with drugs metabolized by CYP2D6 might increase drug levels and increase the risk of adverse effects.

Here's the gist with these two. Your body uses several liver enzymes to clear olanzapine, and one of them is called CYP2D6. Berberine appears to slow that enzyme down a bit. Since olanzapine is an active drug (not a prodrug that needs switching on), slowing its breakdown could let a little more linger in your bloodstream than usual.

In plain terms, that might mean stronger side effects, like feeling extra drowsy or sedated, dizzy when you stand up, or a touch foggy. I'll be honest though: this is mostly theoretical, based on lab work and weak clinical data, and CYP2D6 is only a minor route for olanzapine. So the risk is modest. Still, please run it by your pharmacist or doctor before adding berberine.

About Olanzapine

What Is Olanzapine Used For?

Brand name: Zyprexa

Olanzapine treats schizophrenia and the manic or mixed episodes of bipolar I disorder in both adults and adolescents (ages 13–17). It can also be used alongside valproate or lithium to treat manic or mixed episodes in adults. When combined with fluoxetine, olanzapine tablets and orally disintegrating tablets are used to treat depressive episodes associated with bipolar I disorder and treatment-resistant depression in adults.

Olanzapine intramuscular injection (short-acting) is used by healthcare providers to quickly calm acute agitation in adults with schizophrenia or bipolar I mania. Zyprexa Relprevv — a separate, long-acting intramuscular formulation available only through a restricted program — is indicated specifically for the treatment of schizophrenia in adults.
