Atropine, morphine sulfate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Black Cohosh interacts with.

Some research suggests that black cohosh might inhibit CYP2D6, but there is conflicting evidence.
Some clinical research suggests that black cohosh might modestly inhibit CYP2D6 and increase levels of drugs metabolized by this enzyme. However, contradictory clinical research shows a specific black cohosh product (Remifemin, Enzymatic Therapy) 40 mg twice daily does not significantly inhibit metabolism of a CYP2D6 substrate in healthy study volunteers. Until more is known, use black cohosh cautiously in patients taking drugs metabolized by CYP2D6.

Here's the gist for this combo. Black cohosh might mildly slow down CYP2D6, one of the liver enzymes that helps process certain medicines. Morphine isn't a prodrug, so if its breakdown were slowed, you could in theory see a bit more morphine effect: think extra drowsiness, constipation, or slowed breathing. Atropine doesn't really lean on CYP2D6, so I wouldn't expect much there.

Honestly, the evidence here is shaky and conflicting. One study even found a popular black cohosh product had no real effect. So this is more of a 'be aware' than a 'be alarmed' situation. If you take morphine regularly and want to try black cohosh, just check with your pharmacist or doctor first.
