Here's the thing to know about Bosentan (Tracleer): it can be tough on your liver all by itself. That's why people on it get regular blood tests to keep an eye on liver enzymes. Black cohosh has its own rare reports of liver trouble, including liver failure and a type of liver inflammation. So the worry is simple, you'd be putting two things that can stress the liver together, which may raise the odds of a problem.

To be fair, this is mostly theoretical and based on scattered case reports, not strong studies. Still, since Bosentan already needs liver monitoring, I'd play it safe. Watch for signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and talk with your doctor or pharmacist before adding black cohosh.

Bosentan also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).

Black cohosh may inhibit one form of OATP, OATP2B1, which could reduce the bioavailability and clinical effects of OATP2B1 substrates.
In vitro research shows that black cohosh modestly inhibits OATP2B1. OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds into the body. Inhibition of OATP may reduce the bioavailability of oral drugs that are substrates of OATP.

Here's the gist for Bosentan (Tracleer). Your gut uses little transporters called OATPs to pull certain medicines from your intestine into your bloodstream. Lab studies suggest black cohosh may modestly block one of these, OATP2B1. Since bosentan relies partly on this kind of uptake, the theory is that black cohosh could slightly lower how much bosentan you actually absorb, which in turn could make it work a bit less well for your lung blood pressure.

I want to be straight with you: this is based only on test-tube research, not real patients, so it's rated minor and far from certain. Still, bosentan is important medicine, and you don't want its effect quietly weakened. Please run this by your pharmacist or doctor before pairing the two.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
