Why this applies to Galantamine

Galantamine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Black Cohosh interacts with.

Some research suggests that black cohosh might inhibit CYP2D6, but there is conflicting evidence.
Some clinical research suggests that black cohosh might modestly inhibit CYP2D6 and increase levels of drugs metabolized by this enzyme. However, contradictory clinical research shows a specific black cohosh product (Remifemin, Enzymatic Therapy) 40 mg twice daily does not significantly inhibit metabolism of a CYP2D6 substrate in healthy study volunteers. Until more is known, use black cohosh cautiously in patients taking drugs metabolized by CYP2D6.

Here's the gist for galantamine: your body partly relies on a liver enzyme called CYP2D6 to clear it out. Black cohosh might slow that enzyme down a little, which could let more galantamine linger in your system. Since galantamine works as-is (it's not a prodrug that needs activating), more of it usually means a stronger effect and a better chance of side effects like nausea, vomiting, diarrhea, dizziness, a slower heartbeat, or loss of appetite.

That said, the evidence here is shaky and conflicting. One study even found a popular black cohosh product (Remifemin) didn't meaningfully affect CYP2D6. So this is a possible, modest concern, not a sure thing. Before combining them, check with your pharmacist or doctor so they can keep an eye on how you feel.
