Acetaminophen, Hydrocodone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Black Hoof Mushroom interacts with.
In vitro, polysaccharides from black hoof mushroom grown in culture inhibit CYP1A2. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your Vicodin or Norco (the acetaminophen plus hydrocodone combo). In lab dishes, compounds from black hoof mushroom slowed down a liver enzyme called CYP1A2. The thinking is that if this enzyme runs slower, certain drugs it handles could hang around a bit longer and feel stronger.
Honestly though, hydrocodone is mainly processed by other enzymes (CYP3A4 and CYP2D6), not CYP1A2, so this particular pathway isn't the big player here. And this was only seen in test tubes, never in real people. I wouldn't lose sleep over it. Still, if you ever notice extra drowsiness, dizziness, or slowed breathing, that's worth a quick call to your pharmacist or doctor to be safe.

Here's the gist for your Norco or Vicodin (acetaminophen with hydrocodone). The acetaminophen part is partly handled by a liver enzyme called CYP2E1. In test-tube studies, compounds from black hoof mushroom seem to slow that enzyme down a bit. The theory is that if CYP2E1 is slowed, slightly less acetaminophen gets pushed down a pathway that can form a harmful byproduct, which actually isn't a bad thing for your liver.
The honest truth is that this is only lab data, never shown in real people, so I wouldn't lose sleep over it. The much bigger acetaminophen concern is simply not exceeding the daily limit. Before adding the mushroom, just run it past your pharmacist or doctor, especially if you drink alcohol or have liver concerns.
