Here is the interaction we found between these two, with the evidence behind it.
ANTIDIABETES DRUGS
Why this applies to Semaglutide
Semaglutide falls under ANTIDIABETES DRUGS — the group Black Pepper interacts with.
Animal research shows that piperine, a constituent of black pepper, can reduce blood glucose levels. Monitor blood glucose levels closely. Dose adjustments might be necessary.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind if you take semaglutide (Ozempic, Wegovy, or Rybelsus) and you're thinking about adding a concentrated black pepper or piperine supplement. Semaglutide already lowers blood sugar by helping your pancreas release insulin at the right times. Lab and animal studies suggest piperine, the active part of black pepper, may nudge blood sugar down a little too. Stack the two and, in theory, your readings could dip lower than expected.
I want to be honest about how soft this evidence is. It's theoretical, based mostly on animal research, so I wouldn't lose sleep over it. The pinch of pepper on your dinner won't matter. With supplement-strength piperine, just keep an eye on your numbers for shakiness, sweating, or lightheadedness, and check with your pharmacist or doctor before starting it.

From our pharmacist’s overview on the Semaglutide page
“Semaglutide (Ozempic, Rybelsus, Wegovy) lowers blood sugar, and so do most of the 256 supplements on this page, which is why I read this list with real vigilance rather than a shrug. Nearly all of the entries either add to its glucose-lowering effect and raise the risk of hypoglycemia, or push sugar the other way and blunt it, as niaci…”

About Black Pepper
What is Black Pepper?
- Flavoring and culinary use
- Digestive complaints like bloating and indigestion
- Boosting absorption of other supplements
- Antioxidant and anti-inflammatory support
- Cold and congestion remedies
Black pepper is a common kitchen spice that is generally safe in the amounts used in food. Its extract, piperine, is mostly added to supplements to help the body absorb other ingredients (like curcumin from turmeric). Strong human evidence for black pepper treating specific health conditions is limited, so use supplement-strength products with care and check with your pharmacist.
About Semaglutide
What Is Semaglutide Used For?
Brand names: Ozempic, Rybelsus, Wegovy
Semaglutide is available as Wegovy (injection and tablet) and Ozempic (injection), and each product has its own approved uses. Wegovy injection is approved — alongside diet and exercise — to reduce excess body weight in adults and in children aged 12 and older with obesity, to lower the risk of serious heart events (heart attack, stroke, or cardiovascular death) in adults with obesity or overweight who also have established heart disease, and for treatment of a liver condition called MASH (metabolic dysfunction-associated steatohepatitis, formerly called NASH) with moderate-to-advanced scarring in adults. Wegovy tablets share the cardiovascular risk reduction and weight management uses in adults, but are not approved for MASH or for use in children. Ozempic injection is approved to improve blood sugar control in adults with type 2 diabetes (alongside diet and exercise) and to reduce the risk of major cardiovascular events in adults with type 2 diabetes who also have established heart disease.
All forms are used alongside a reduced-calorie diet and increased physical activity. Ozempic is not approved for type 1 diabetes. The MASH approval for Wegovy injection is an accelerated approval, meaning continued approval may depend on results from a confirmatory study.

Semaglutide Is Linked to Lower Levels of One Nutrient
Natural Medicines records Semaglutide as a medicine that can lower these. This is not an interaction with Black Pepper — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
