Black Psyllium and Irbesartan

Here is the interaction we found between these two, with the evidence behind it.

Minor

ORAL DRUGS

Why this applies to Irbesartan

Irbesartan falls under ORAL DRUGS — the group Black Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin. Although many of these studies evaluated blond psyllium, the fiber content in black psyllium may have similar effects.
Severity Minor Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: black psyllium is a soluble fiber that turns into a thick gel in your gut. That gel can sometimes grab onto a pill and slow or slightly change how much of it gets absorbed. With Irbesartan, your blood pressure medicine, this is mostly a theoretical concern, and the evidence is thin. There's no specific study showing a problem with irbesartan itself.

If the gel did blunt absorption a bit, you might get slightly less blood pressure control. An easy fix that sidesteps the whole issue: take your irbesartan first, then take the psyllium about 30 to 60 minutes later, always with plenty of water. Keep an eye on your home blood pressure readings, and check with your pharmacist or doctor before starting the fiber.

Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

About Irbesartan

What Is Irbesartan Used For?

Brand names: Avapro, Aprovel

Irbesartan tablets — sold as the brand Avapro and as generic irbesartan — are used to treat high blood pressure (hypertension). Bringing blood pressure down lowers the risk of serious events like stroke and heart attack. It can be used alone or alongside other blood pressure medicines.

Irbesartan tablets (including Avapro) are also used to treat diabetic nephropathy — kidney disease caused by diabetes — in people who have type 2 diabetes, high blood pressure, and signs of kidney damage. In this group, it helps slow how quickly the kidney disease gets worse.
