Imipramine falls under SEROTONERGIC DRUGS — the group Black Seed interacts with.

Theoretically, combining serotonergic drugs with black seed might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders.
Animal research suggests that black seed can increase brain serotonin levels. In one case report, a 35-year-old man undergoing endoscopic surgery experienced immediate postoperative serotonin syndrome that was likely associated with the use of black seed oil 600 mg daily starting 4 days before surgery, and precipitated by the use of serotonergic pain medications, including fentanyl and oxycodone. Monitor patients for signs of serotonin syndrome and other serotonergic side effects if using black seed with serotonergic drugs.

Here's the gist: imipramine raises serotonin levels in your brain to help with depression, and there's a theory that black seed may nudge serotonin up a bit too. Put the two together and you could, in rare cases, get more serotonin activity than you want. The signs to watch for include feeling agitated or restless, a racing heart, sweating, shaking, muscle twitches, or confusion.

I want to be honest about the evidence, though. This is mostly based on animal studies and a single case report, so it's more of a 'be aware' than a 'this will happen.' A sprinkle of black seed as a spice almost certainly won't matter; concentrated oil capsules are the bigger question mark. Before adding a black seed supplement, run it by your pharmacist or doctor.

Theoretically, black seed might increase levels of drugs metabolized by CYP2C9.
In vitro research suggests that thymoquinone, a constituent of black seed, can decrease the metabolism of phenytoin by a mechanism possibly related to the inhibition of CYP2C9. The effect of black seed on CYP2C9 is unclear. This has not been shown in humans.

Here's the idea behind this pairing. Black seed, mainly its compound thymoquinone, might slow down a liver enzyme called CYP2C9, which is one of the pathways that helps clear imipramine (Tofranil) from your body. If that pathway slows, imipramine could build up a bit higher than expected. With more drug hanging around, you might notice stronger side effects like drowsiness, dry mouth, constipation, blurry vision, dizziness when standing, or a faster heartbeat.

I want to be honest, though: this is only a theoretical concern based on lab work, not something proven in real people, and black seed's true effect on this enzyme is still unclear. Food-amount seasoning is unlikely to matter much; concentrated supplement doses are the bigger question mark. Please check with your pharmacist or doctor before combining them.

What Is Imipramine Used For?

Brand names: Tofranil, Tofranil PM

Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.

Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
