Isosorbide Dinitrate, Hydralazine falls under ANTIHYPERTENSIVE DRUGS — the group Black Seed interacts with.

Theoretically, taking black seed with antihypertensive drugs might increase the risk of hypotension.
Clinical research suggests that black seed powder and oil might reduce blood pressure by 2-3 mmHg. In animal research, black seed modestly reduces blood pressure and concomitant use of black seed and amlodipine (Norvasc) or metoprolol (Lopressor) increased the blood pressure lowering effects of these drugs.

BiDil already does two jobs to lower blood pressure: the isosorbide dinitrate relaxes and widens your blood vessels, while hydralazine eases the workload on your heart. Black seed seems to nudge blood pressure down a little too, by maybe 2 to 3 mmHg in studies. Stack that small effect on top of BiDil and you could end up lower than intended.

What that looks like in real life is feeling dizzy, lightheaded, or faint, especially when you stand up quickly, along with possible headache or fatigue. This is mostly theoretical and based on modest research, and food-amount black seed in cooking is unlikely to matter. Still, concentrated supplements are stronger, so check with your pharmacist or doctor before starting one.
