Why this applies to Bedaquiline

Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bog Labrador Tea interacts with.

In vitro research shows that aqueous and ethanolic extracts of Bog Labrador tea leaves can increase the concentration of cytochrome P450 3A4 (CYP3A4) enzymes. Extracts obtained from different geographical regions in Quebec resulted in differing potency of CYP3A4 induction. Bog Labrador tea might increase the metabolism of CYP3A4 substrates.
Drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), amiodarone (Cordarone), ticagrelor (Brilinta), triazolam (Halcion), and others.

Here's the gist for Bedaquiline (Sirturo). Your body clears this drug using a liver enzyme called CYP3A4. Lab studies suggest Bog Labrador tea may rev that enzyme up (induce it), which could speed up how fast bedaquiline is removed from your body. In plain terms, that might mean lower blood levels and a weaker drug, and with a serious infection like multi-drug resistant TB, you really don't want your antibiotic working at half strength.

I want to be honest though: this is based only on test-tube research, not real people, and the effect varied a lot between plant samples. So it's a theoretical concern, not a proven one. Because completing TB treatment matters so much, please run this by your pharmacist or doctor before adding the tea.

Bog Labrador tea is a wetland shrub whose leaves have long been brewed as a traditional beverage and folk remedy, especially among Indigenous peoples of North America. There is very little human research to confirm any health benefit, and the plant is closely related to species that contain toxic compounds, so it should be used cautiously and in moderation. Pregnant or breastfeeding people should avoid it, and anyone considering it should talk with a pharmacist or doctor first.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
