Treprostinil (Inh) falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Boldo interacts with.

Theoretically, taking boldo with anticoagulant/antiplatelet drugs might increase the risk of bleeding.
Animal and in vitro research shows that boldine, a constituent of boldo, has antiplatelet activity. In one case report, an adult taking a combination of boldo and fenugreek with warfarin experienced an increase in international normalized ratio (INR); however, it is unclear if this effect was due to boldo, fenugreek, the combination, or another factor.

Here's the connection worth knowing about. Inhaled treprostinil (Tyvaso) does more than relax the blood vessels in your lungs. It also makes platelets a little less sticky, so it gently thins the blood on its own. Boldo may nudge in the same direction, since lab and animal studies suggest one of its compounds (boldine) can also reduce platelet clumping. Stack the two and you could, in theory, bruise more easily or bleed a bit longer from a cut.

I want to be honest about how thin the evidence is here. This is mostly test-tube and animal data plus one messy case report, so it's more of a caution than a proven problem. Still, before you add a boldo supplement, run it past your pharmacist or doctor first.

Boldo is a South American shrub whose leaves are traditionally used for digestive and gallbladder complaints, but good human evidence is very limited. The leaves and oil contain ascaridole, which can be toxic to the liver, so only ascaridole-free products should be used and only for short periods. Talk to your pharmacist or doctor before trying it, and avoid it during pregnancy and breastfeeding.

What Is Treprostinil Oral Inhalation Used For?

Brand name: Tyvaso

Treprostinil oral inhalation is used to treat two serious lung conditions that raise pressure in the blood vessels of the lungs. It helps improve how much physical activity you can do.

Tyvaso inhalation solution and Tyvaso DPI are both indicated to treat pulmonary arterial hypertension (PAH; WHO Group 1) and pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3). Clinical studies establishing effectiveness in PAH primarily included patients with moderate symptoms (NYHA Class III) whose PAH was idiopathic, heritable, or related to connective tissue disease. Studies in PH-ILD primarily included patients with idiopathic interstitial pneumonia, combined pulmonary fibrosis and emphysema, or connective tissue disease-related lung disease.
