Codeine Phosphate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase the levels of CYP2D6 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP2D6 enzymes.

Here's the twist with codeine: it's actually a prodrug. Your body has to convert it into morphine using an enzyme called CYP2D6 before it can ease your pain or quiet a cough. Boswellia, in lab studies, looks like it might slow that same enzyme down a bit.

So if Boswellia blocks CYP2D6, your body may convert less codeine into morphine. That could mean weaker pain relief or a cough that isn't well controlled. The flip side is that this is only theoretical, based on test-tube research, not real patients, so the effect may be small or not noticeable at all. If you take both and feel your codeine isn't working as well, check in with your pharmacist or doctor rather than adjusting anything yourself.

Codeine Sulfate tablets are used for the management of mild to moderate pain in adults and adolescents when other treatments have not worked well enough or are not tolerated. It is an opioid medication, so it is only appropriate when non-opioid options are inadequate.

Because of its serious risks — including addiction, misuse, and overdose — codeine is reserved for patients who truly need an opioid. Prescribers are expected to use the lowest effective dose for the shortest time possible.
