Copanlisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Here's the short version on pairing boswellia with your Copanlisib (Aliqopa) infusions. Copanlisib is handled in the body partly by a liver enzyme called CYP3A4. In lab dish studies, boswellic acids mostly slow that enzyme down (and may speed it up a little at very low amounts). If it slows Copanlisib's breakdown, more of the drug could linger, which might mean stronger side effects like high blood sugar, high blood pressure, low blood counts, or infections.

I want to be honest though: this is theoretical, based only on test-tube research, not on real patients. Copanlisib is a serious cancer therapy, so don't add boswellia on your own. Please run it by your oncology team or pharmacist first so they can weigh in.

Theoretically, Boswellia serrata might alter the effects of immunosuppressive drugs.
Some in vitro research suggests that Boswellia serrata extracts might inhibit mediators of autoimmune disorders such as leukotrienes and reduce production of antibodies and cell-mediated immunity. However, other in vitro research suggests that, when coupled with calcium ions, boswellic acids containing the keto group have immunostimulant properties within specific cell signaling pathways.

Here's the honest picture: Copanlisib (Aliqopa) is a cancer medicine that works in part by quieting overactive immune signaling. Boswellia, the resin used for joint aches, seems to nudge the immune system too, but the catch is that lab studies disagree on which direction it pushes. Some test-tube work suggests it calms immune activity, while other data hint it can actually stimulate it depending on the cell pathway involved.

Because of that uncertainty, we just can't predict whether adding Boswellia would blunt or amplify what Copanlisib is doing. Keep in mind this is theory from lab and animal work, not from people taking the two together. Still, with a serious cancer treatment, I wouldn't add Boswellia on your own. Please run it by your oncologist or pharmacist first.
