These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Boswellia Serrata interacts with whole categories of medication. Dextromethorphan HBr, Phenylephrine HCl falls into 2 of those categories, so it can interact with Boswellia Serrata in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
1 of 2Why this applies to Dextromethorphan HBr, Phenylephrine HCl
Dextromethorphan HBr, Phenylephrine HCl falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Boswellia Serrata interacts with.
In vitro research shows that Boswellia serrata gum resin inhibits CYP2D6 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: the dextromethorphan in this cough and congestion product is cleared from your body mainly by a liver enzyme called CYP2D6. In lab testing, boswellic acids appear to slow that enzyme down. If that happens in real life, dextromethorphan could build up a bit and lead to stronger effects like drowsiness, dizziness, or feeling foggy or lightheaded.
I want to be honest about how soft this evidence is. It comes only from test-tube studies, not from people, so it may not amount to much in everyday use, especially with short-term cough relief. The phenylephrine piece is not really involved here. If you take boswellia regularly, just mention it to your pharmacist or doctor before pairing it with this product.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Dextromethorphan HBr, Phenylephrine HCl also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Vicks Formula 44D, which pairs dextromethorphan (the cough suppressant) with phenylephrine (the decongestant). Lab studies suggest boswellia may slow down a liver enzyme called CYP3A4, one of the pathways that helps break dextromethorphan down. If that clearing slows, a little more dextromethorphan could linger in your system, which might mean more drowsiness, dizziness, or feeling foggy than usual.
I want to be honest: this is purely theoretical, based on test-tube research, not real people taking these together. For most folks using this for a short cold, it's unlikely to cause trouble. Still, if you take boswellia regularly or feel extra groggy, mention it to your pharmacist or doctor before combining them.
