Leniolisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Here's the situation with pairing Boswellia serrata and leniolisib (Joenja). Your body clears leniolisib using a liver enzyme called CYP3A4. Leniolisib is an active drug, not a prodrug, so it doesn't need this enzyme to switch on and start working.

In lab (test tube) studies, boswellic acids mostly slow down CYP3A4, which could theoretically let leniolisib build up a bit and raise the chance of side effects. Interestingly, at very low amounts it may speed the enzyme up instead, which could weaken the drug. I want to be honest: this is theoretical and hasn't been shown in real people, so I wouldn't lose sleep over it. Still, since leniolisib treats a serious immune condition, please loop in your pharmacist or doctor before adding boswellia so they can keep an eye on you.

Joenja (leniolisib) is approved to treat activated phosphoinositide 3-kinase delta syndrome, commonly called APDS — a rare genetic immune disorder in which certain immune cells (B and T cells) don't work properly. It is approved for adults and pediatric patients who are at least 12 years old.

APDS is caused by gene variants that make a specific enzyme (PI3K-delta) overactive, leading to immune system dysfunction. Joenja is the brand name for leniolisib in tablet form, taken by mouth.
