Here's the gist for your Namzaric (which combines donepezil and memantine). Donepezil is partly handled by a liver enzyme called CYP2D6. In test-tube studies, boswellia appears to slow that enzyme down. If that happened in your body, donepezil could build up a bit and lead to stronger effects, like nausea, diarrhea, loss of appetite, dizziness, or a slower heart rate. Memantine isn't really cleared by CYP2D6, so that part is less of a concern.

I want to be honest, though: this is purely theoretical, based on lab research, not on real people. Neither of these is a prodrug, so the direction stands as written. Still, it's worth a quick word with your pharmacist or doctor before adding boswellia.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Namzaric combines two Alzheimer's medicines, memantine and donepezil. Of the two, donepezil is the one that leans on the liver enzyme CYP3A4 to be cleared, while memantine is mostly passed out through your kidneys largely unchanged. So this interaction really applies to the donepezil piece.

Here's the honest part: this is a theoretical concern based only on lab dish studies, not real people. In those studies, Boswellia mostly slowed CYP3A4, which could in theory let donepezil build up a little and bring out more of its usual effects, things like nausea, diarrhea, slower heart rate, dizziness, or muscle cramps. The effect was inconsistent, so I wouldn't lose sleep over it. Still, since these are sensitive meds, mention any Boswellia supplement to your pharmacist or doctor before starting.
