Theoretically, Boswellia serrata might increase the levels of CYP1A2 substrates.

Here's the gist for Esbriet (pirfenidone): your liver relies on an enzyme called CYP1A2 to clear this medicine out of your body. In test-tube studies, boswellia appears to slow that same enzyme down. If that happens in real life, pirfenidone could build up a bit higher than usual, which may mean more of its typical side effects, things like nausea, upset stomach, tiredness, dizziness, or sensitivity to sunlight.

I want to be honest, though: this concern comes only from lab research, not from studies in people, so it's more of a theoretical caution than a proven problem. Pirfenidone is not a prodrug, so this would lean toward stronger effects, not weaker ones. Before adding boswellia, just run it by your pharmacist or doctor so they can weigh in for your situation.
