Lepirudin falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Bupleurum interacts with.

Theoretically, bupleurum might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
In vitro research suggests that saikosaponins, constituents of bupleurum, can inhibit platelet aggregation.

Lepirudin is a powerful injectable blood thinner, often used in the hospital for people who can't take heparin. It works directly to block clotting, so your blood is already slower to clot while you're on it. Bupleurum is a different story: in lab tests, its saikosaponin compounds appear to make platelets (the cells that help form clots) stick together less easily.

Stacking those two effects together could, in theory, raise your bleeding risk a bit. Watch for things like easy bruising, nosebleeds, bleeding gums, or blood in your urine or stool. To be clear, this concern comes only from lab research, not from real patient studies, so it's more of a caution than a proven problem. Since lepirudin is given in a closely monitored hospital setting, please mention any herbal products to your care team rather than changing anything yourself.
