Cabbage and Cabotegravir Sodium

Here is the interaction we found between these two, with the evidence behind it.

Moderate

GLUCURONIDATED DRUGS

Why this applies to Cabotegravir Sodium

Cabotegravir Sodium falls under GLUCURONIDATED DRUGS — the group Cabbage interacts with.

Theoretically, cabbage might increase clearance and decrease the effects of drugs metabolized through glucuronide conjugation.
A small clinical study shows that daily consumption of cabbage and Brussels sprout decreases levels of some drugs metabolized through glucuronide conjugation.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the connection worth knowing: your body clears cabotegravir mainly by attaching a sugar molecule to it, a process called glucuronidation. Cabbage, especially in large or concentrated amounts, may speed that process up a bit. In theory, that could lower cabotegravir levels in your blood, and with an HIV medication, keeping levels steady is what keeps the virus controlled.

The good news is that this is based on a small study and is still mostly theoretical, so normal food portions of cabbage are very unlikely to cause a problem. I wouldn't lose sleep over a side of coleslaw. If you're thinking about eating large daily amounts or taking concentrated cabbage supplements, that's the time to check with your pharmacist or HIV provider first. Please don't change your medication on your own.

From our pharmacist’s overview on the Cabotegravir Sodium page

“Cabotegravir (Vocabria) is an HIV medicine, and keeping its blood level steady is what keeps the virus suppressed, so I read this list of 29 interacting supplements with genuine concern. The one major entry is activated charcoal, a popular detox remedy that soaks up drugs in the gut and cuts how much reaches you.”

About Cabbage

What is Cabbage?

Cabbage
  • Breast engorgement during breastfeeding
  • Joint pain and swelling (topical)
  • Digestive and stomach upset
  • General nutrition and antioxidant support
  • Skin and minor inflammation (topical)

Cabbage is a nutritious, low-calorie vegetable that is safe to eat as food and is sometimes applied to the skin as a leaf wrap for breast engorgement or sore joints. Most medicinal claims are based on tradition or limited research, with the strongest (though still modest) evidence being for cooling cabbage leaves on engorged breasts. Talk to your pharmacist or doctor before using cabbage in large amounts or as a treatment.

About Cabotegravir

What Is Cabotegravir Used For?

Brand name: Vocabria

Vocabria (cabotegravir) tablets are used in combination with rilpivirine (Edurant) to treat HIV-1 infection in adults and adolescents aged 12 and older who weigh at least 35 kg and whose HIV is already virologically suppressed (virus levels undetectable or very low on their current regimen). Vocabria tablets serve as a short-term oral lead-in — taken for roughly one month — before switching to long-acting injectable therapy.

Cabotegravir tablets are also used as an oral lead-in for HIV-1 pre-exposure prophylaxis (PrEP) — helping HIV-negative adults and adolescents (weighing at least 35 kg) confirm they tolerate the drug before starting injectable PrEP. Both uses require confirmed HIV-negative status before starting.
