Conjugated Estrogens falls under GLUCURONIDATED DRUGS — the group Cabbage interacts with.
A small clinical study shows that daily consumption of cabbage and Brussels sprout decreases levels of some drugs metabolized through glucuronide conjugation.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist for Premarin and other conjugated estrogens: your body partly clears these hormones through a process called glucuronidation. Cabbage (and its cousin Brussels sprouts) may speed that cleanup pathway up a bit. In a small human study, eating these veggies daily lowered levels of some drugs handled this way. So in theory, regularly eating large amounts of cabbage could nudge your estrogen levels down and make the medicine work a little less well, meaning hot flashes, night sweats, or vaginal dryness might creep back.
I want to keep this in perspective. This is a modest, mostly theoretical concern, and normal amounts of cabbage in meals are very unlikely to matter. It would take consistent, heavy intake to see much effect. If you're loading up on cabbage or taking concentrated supplements, just mention it to your pharmacist or doctor before changing anything.
