Cabbage and Deferasirox

Here is the interaction we found between these two, with the evidence behind it.

Moderate

GLUCURONIDATED DRUGS

Why this applies to Deferasirox

Deferasirox falls under GLUCURONIDATED DRUGS — the group Cabbage interacts with.

Theoretically, cabbage might increase clearance and decrease the effects of drugs metabolized through glucuronide conjugation.
A small clinical study shows that daily consumption of cabbage and Brussels sprout decreases levels of some drugs metabolized through glucuronide conjugation.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with deferasirox (Exjade, Jadenu). Your body clears this iron chelator partly through a process called glucuronidation, basically tacking on a chemical tag so the drug can be removed. There's some early human research suggesting that eating cabbage (and Brussels sprouts) regularly can speed up that tagging process, which could lower deferasirox levels and make it work a little less well at pulling excess iron out of your body.

Now, a normal serving of cabbage at dinner is very unlikely to matter. This signal came from daily, fairly heavy intake, and the evidence is still modest and partly theoretical. Since deferasirox is treating real iron overload, I wouldn't change your diet or your dose on your own. Just mention your cabbage habits to your doctor or pharmacist so they can keep an eye on your iron levels.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.

About Cabbage

What is Cabbage?

Cabbage
  • Breast engorgement during breastfeeding
  • Joint pain and swelling (topical)
  • Digestive and stomach upset
  • General nutrition and antioxidant support
  • Skin and minor inflammation (topical)

Cabbage is a nutritious, low-calorie vegetable that is safe to eat as food and is sometimes applied to the skin as a leaf wrap for breast engorgement or sore joints. Most medicinal claims are based on tradition or limited research, with the strongest (though still modest) evidence being for cooling cabbage leaves on engorged breasts. Talk to your pharmacist or doctor before using cabbage in large amounts or as a treatment.

About Deferasirox

What Is Deferasirox Used For?

Brand names: Exjade, Jadenu

Deferasirox — available as tablets (film-coated and for oral suspension) and as oral granules, and sold under brand names including Exjade (tablet for oral suspension) and Jadenu (film-coated tablet and granules) — is used to remove excess iron that builds up in the body. All formulations are approved to treat chronic iron overload due to blood transfusions in patients 2 years of age and older.

All formulations are also approved for chronic iron overload in non-transfusion-dependent thalassemia (NTDT) syndromes in patients 10 years of age and older, but only when liver iron levels and serum ferritin meet specific thresholds. Using deferasirox together with other iron-chelation medicines has not been shown to be safe or effective.
