Pirfenidone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Cabbage interacts with.

Theoretically, cabbage might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that cabbage or its constituent indole-3-carbinol might increase drug metabolism and elimination by stimulating CYP1A2 activity.

Here's the gist for Esbriet (pirfenidone). Your liver clears this medicine using an enzyme called CYP1A2. Cabbage, thanks to plant compounds like indole-3-carbinol, may gently nudge that enzyme to work faster. If that happens, your body could break down and remove pirfenidone a little quicker, which in theory means less drug in your system and possibly a weaker effect on slowing your lung scarring.

The good news: this is mostly a theory based on lab and animal studies, not solid human data. Normal amounts of cabbage in your meals are very unlikely to cause trouble. I'd only be cautious with concentrated cabbage supplements or large daily portions. Before adding those, just run it by your pharmacist or doctor.

Pirfenidone — available as capsules and tablets under the brand name Esbriet, as well as generic pirfenidone products — is indicated for the treatment of idiopathic pulmonary fibrosis (IPF), a chronic, progressive disease in which scar tissue builds up in the lungs and makes breathing increasingly difficult.

Esbriet capsules and tablets, pirfenidone capsules, and pirfenidone tablets are each specifically approved for IPF. No other uses are supported by the available product labels.
