Acetaminophen, Caffeine, Chlorpheniramine, Hydrocodone, Phenylephrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Cabbage interacts with.
Some animal research suggests that cabbage or its constituent indole-3-carbinol might increase drug metabolism and elimination by stimulating CYP1A2 activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination product packs a lot in, but the piece that touches cabbage is mostly the caffeine. Caffeine is cleared by an enzyme called CYP1A2. In theory, eating large amounts of cabbage (or concentrated cabbage extracts) might speed that enzyme up a bit, so your body could clear caffeine a little faster. In plain terms, the caffeine in this medicine might feel slightly less noticeable, meaning less of that jittery or wide-awake kick.
Honestly, I wouldn't lose sleep over this one. The evidence comes only from lab and animal studies, so it's theoretical, and normal dinner-sized servings of cabbage are very unlikely to matter. If you're eating big amounts daily or using supplements, just mention it to your pharmacist or doctor.

This combo product (Hycomine Compound) pairs acetaminophen (Tylenol) with caffeine, the antihistamine chlorpheniramine, the opioid hydrocodone, and the decongestant phenylephrine. The piece that matters here is the acetaminophen, since a good chunk of it is cleared by your liver through a process called glucuronidation. Eating a lot of cabbage (and other cruciferous veggies like Brussels sprouts) may speed up that pathway a little, which could slightly lower acetaminophen levels and shave off some of its pain and fever relief.
This is a modest, mostly theoretical concern based on small studies, so don't panic. Normal dinner-sized servings of cabbage are very unlikely to be a problem. If you're loading up on cabbage daily or as a supplement, just mention it to your pharmacist or doctor before changing anything.

This combination product contains acetaminophen for pain and fever, and that's the piece cabbage may touch. In one small study, eating cabbage and Brussels sprouts every day sped up how the body clears acetaminophen, lowering its blood levels by up to about 16%. The likely reason is that cabbage nudges the liver to attach acetaminophen to a sugar molecule (glucuronide) and flush it out faster.
In plain terms, lots of cabbage could make the acetaminophen part work a little less, so your pain or fever relief might not last quite as long. This is a modest effect, and normal amounts of cabbage in meals are unlikely to cause trouble. It doesn't change the hydrocodone, caffeine, antihistamine, or decongestant pieces. Don't change your dose on your own, just check with your pharmacist or doctor if you're eating cabbage in large or supplement-sized amounts.
