Ropeginterferon alfa-2b-njft falls under PHOTOSENSITIZING DRUGS — the group Chlorella interacts with.

Theoretically, chlorella might have additive effects with photosensitizing drugs.
Chlorella has been reported to cause photosensitization. In five case reports, patients who had ingested chlorella exhibited swelling followed by erythematopurpuric lesions on sun-exposed areas of the body. Theoretically, concomitant use with photosensitizing drugs may exacerbate effects.

Here's the connection in plain terms. Besremi (ropeginterferon) can make your skin more sensitive to sunlight, and chlorella has its own track record of doing the same thing. A handful of case reports describe people who took chlorella and then developed swelling and reddish-purple spots on sun-exposed skin. Put the two together and, in theory, you could sunburn or react more easily than usual.

I want to be honest that this is a theoretical concern based on how each one behaves, not a proven, common problem. It isn't about chlorella changing your drug levels. Still, if you take both, be smart about sun protection (sunscreen, hats, covered skin) and watch for any unusual rash. Check with your pharmacist or doctor before adding chlorella while on Besremi.

BESREMi (ropeginterferon alfa-2b) is approved to treat two blood disorders in adults: polycythemia vera — a condition where the bone marrow makes too many red blood cells — and essential thrombocythemia — a condition where the bone marrow makes too many platelets.

Both conditions are treated with BESREMi given as a subcutaneous (under the skin) injection.
