Chokeberry and Aspirin, Caffeine, Hydrocodone

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Chokeberry interacts with whole categories of medication. Aspirin, Caffeine, Hydrocodone falls into 2 of those categories, so it can interact with Chokeberry in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

1 of 2

Why this applies to Aspirin, Caffeine, Hydrocodone

Aspirin, Caffeine, Hydrocodone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chokeberry interacts with.

Theoretically, chokeberry might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that chokeberry inhibits CYP3A4. In humans, there is one case report of a drug interaction with trabectedin, a CYP3A4 substrate, which is hypothesized to have been caused by chokeberry inhibition of CYP3A4.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combination product contains hydrocodone, an opioid painkiller that your liver partly processes using an enzyme called CYP3A4. Chokeberry has been shown in a test tube to slow down that same enzyme. In theory, that could let hydrocodone build up a bit more than usual, which might mean stronger drowsiness, more constipation, dizziness, or slowed breathing. The caffeine in this product also leans on CYP3A4, so you could feel a touch more jittery too.

Honestly though, this is very weak evidence. It rests on lab work and a single case report with a different drug, not real-world experience with this medicine. Everyday chokeberry juice or jam is unlikely to be a problem, but check with your pharmacist before using concentrated chokeberry supplements.

ANTICOAGULANT/ANTIPLATELET DRUGS

2 of 2

Aspirin, Caffeine, Hydrocodone also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Theoretically, chokeberry might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
Clinical and in vitro research suggests that chokeberry extract can temporarily inhibit platelet aggregation and decrease clot formation.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the piece that matters with this combo product: the aspirin part already thins your blood by keeping platelets from clumping together. Chokeberry seems to do a little of the same thing. Some lab and small human studies suggest its dark-berry polyphenols can mildly slow platelet activity and clot formation. Stack that on top of aspirin and you may bruise more easily, bleed a bit longer from a cut, or rarely run into stomach bleeding.

This is still mostly theoretical, and a little chokeberry juice or jam is unlikely to cause trouble. Concentrated supplement doses are the bigger question. The caffeine and hydrocodone parts aren't really involved here. Before adding a chokeberry supplement, it's worth a quick chat with your pharmacist or doctor.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.

About Chokeberry

What is Chokeberry?

Chokeberry
  • Antioxidant support
  • Heart and blood vessel health
  • Blood pressure support
  • Blood sugar support
  • Common cold symptoms
  • Anti-inflammatory support

Chokeberry (aronia) is a dark berry rich in antioxidants called polyphenols, and it's widely eaten as juice, jam, and supplements. Early research hints it may support heart health, blood pressure, and blood sugar, but the evidence is still limited and far from proven. It's generally well tolerated as a food, but talk to your pharmacist or doctor before using concentrated supplements.
