simvastatin/ezetimibe falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chokeberry interacts with.

Theoretically, chokeberry might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that chokeberry inhibits CYP3A4. In humans, there is one case report of a drug interaction with trabectedin, a CYP3A4 substrate, which is hypothesized to have been caused by chokeberry inhibition of CYP3A4.

Here's the simple version. The simvastatin part of Vytorin is cleared from your body by a liver enzyme called CYP3A4. Chokeberry, in lab testing, seems to slow that enzyme down a bit. If that happens in real life, simvastatin could build up to higher levels than usual. That can raise the odds of statin side effects like muscle aches, soreness, or weakness, and rarely more serious muscle problems. The ezetimibe piece isn't really affected by this enzyme.

That said, this is mostly theory plus one case report with a different drug, so it's far from proven. Everyday amounts of chokeberry juice or jam are unlikely to be a problem. Before starting a concentrated chokeberry supplement, just check with your pharmacist or doctor.
