Benzhydrocodone, Acetaminophen falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Clove interacts with.
In vitro research shows that eugenol, the principal constituent of clove, can inhibit CYP2D6 in a dose-dependent manner. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with Apadaz: the benzhydrocodone part is a prodrug, meaning your body has to convert it into hydrocodone (and then partly into hydromorphone) before you get real pain relief. The enzyme CYP2D6 helps with some of that activation. In a lab dish, eugenol (the main compound in clove) slowed CYP2D6 down. If that happened in a person, it could mean less of the active painkiller forms, so you might get weaker relief than expected.
Honestly though, this is only test-tube evidence, never shown in real people, so I wouldn't lose sleep over it. A little clove in your cooking won't matter. Concentrated clove oil or capsules are a different story, so just mention them to your pharmacist or doctor.

Here's the gist: clove's main compound, eugenol, can slow down a liver enzyme called CYP1A2 in lab testing. The idea is that if you block that enzyme, certain medications it handles could build up a bit. With your Apadaz (benzhydrocodone with acetaminophen), benzhydrocodone is actually a prodrug, meaning your body has to convert it into the active opioid (hydrocodone). The main activation step doesn't run through CYP1A2, so this particular pathway isn't a major player for how strong your pain relief turns out.
The reassuring part: this is only seen in test tubes, not in real people, and the small amount of clove you'd get cooking is very unlikely to matter. Concentrated clove oil or supplements are a different story. If you use those, run it by your pharmacist or doctor first, and watch for extra drowsiness.

Here's the thing to know about Apadaz: the benzhydrocodone part is actually a prodrug. Your body has to convert it into hydrocodone before it can ease your pain. Lab studies suggest that eugenol, the main compound in clove, can slow down an enzyme called CYP3A4. Since some of that conversion and breakdown involves CYP3A4, large amounts of clove could, in theory, change how much active drug ends up in your system.
If the opioid builds up, you might see stronger effects like drowsiness, dizziness, constipation, nausea, or slowed breathing. I want to be honest, though: this has only been shown in test tubes, never in people, so the real-world risk is likely small. Cooking with clove is no worry. Before using concentrated clove oil or capsules, just check with your pharmacist or doctor.

What Is Benzhydrocodone and Acetaminophen Used For?
Brand name: Apadaz
Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.
Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
