Clove and Codeine Phosphate, Pseudoephedrine, Triprolidine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Why this applies to Codeine Phosphate, Pseudoephedrine, Triprolidine

Codeine Phosphate, Pseudoephedrine, Triprolidine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Clove interacts with.

Theoretically, concomitant use of clove may increase levels of drugs metabolized by CYP2D6.
In vitro research shows that eugenol, the principal constituent of clove, can inhibit CYP2D6 in a dose-dependent manner. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's a wrinkle worth knowing about this combo product. The codeine part is actually a prodrug, meaning your body has to convert it into morphine using an enzyme called CYP2D6 before it can relieve pain or quiet a cough. Lab studies suggest eugenol, the main oil in clove, can slow that same enzyme down. In theory, that could mean less codeine gets turned into its active form, so you might get weaker pain or cough relief rather than more.

That said, this is based only on test-tube research and hasn't actually been seen in people. Sprinkling clove on food is very unlikely to matter. If you're using concentrated clove oil or capsules regularly, just mention it to your pharmacist so you're not surprised by reduced relief.

From our pharmacist’s overview on the Codeine Phosphate, Pseudoephedrine, Triprolidine page

“Alcohol is the standout risk here. Beer and wine both carry major ratings against CoActifed (codeine phosphate, pseudoephedrine, and triprolidine), backed by solid human evidence, because drinking deepens the drowsiness and can dangerously slow breathing on a codeine-containing medicine.”

About Clove

What is Clove?

Clove
  • Toothache and oral pain
  • Digestive upset
  • Antimicrobial and antifungal
  • Cough and respiratory complaints
  • Flavoring in food and drinks

Clove is a common cooking spice that is also used in traditional medicine, especially as a topical numbing agent for tooth pain thanks to its main compound, eugenol. Food amounts are generally safe, but concentrated clove oil and supplements can be irritating or toxic if overused, and human evidence for most health claims is limited. Talk to your pharmacist or doctor before using clove medicinally, especially in children or during pregnancy.
