Macitentan, Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Clove interacts with.

Theoretically, concomitant use of clove may increase levels of drugs metabolized by CYP3A4.
In vitro research shows that eugenol, the principal constituent of clove, can inhibit CYP3A4 in a dose-dependent manner. This effect has not been reported in humans.

Opsynvi is a combo pill of macitentan and tadalafil, both used to treat pulmonary arterial hypertension. Your liver clears both of these medicines using an enzyme called CYP3A4. Here's the theory: a compound in clove called eugenol can slow that same enzyme down, at least in lab dish studies. If it did the same thing in your body, your medicine could clear more slowly and build up a bit.

What might that look like? A stronger blood-pressure-lowering effect, so more dizziness, flushing, headache, low blood pressure, or fluid retention. I want to be honest though: this has only been seen in test tubes, not in people, so it's more of a caution than a proven problem. Cooking with clove is very unlikely to matter. Concentrated clove oil or supplements are the real question. Please check with your doctor or pharmacist before using those regularly.

Opsynvi combines two medicines, macitentan and tadalafil, that partly rely on a liver enzyme called CYP2C9 to be broken down. Lab studies suggest that eugenol, the main compound in clove, can slow that enzyme down. In theory, that could let these drugs build up a bit more in your body. With tadalafil, higher levels might mean more headache, flushing, stuffy nose, or low blood pressure symptoms like dizziness. Since both drugs already lower blood pressure, that is the piece worth watching.

Here is the reassuring part: this comes only from test-tube research, not from people, and normal food amounts of clove are very unlikely to cause trouble. Concentrated clove oil or supplements are the bigger question. Before using those, just check with your pharmacist or doctor.
